At Novel Hospice Care, we understand that deciding on hospice for end-of-life care is a difficult decision. However, referring the patient to hospice as soon as possible after a terminal diagnosis can significantly increase the effectiveness of hospice care because it gives the care team sufficient time to assess/address the patient’s physical, emotional and spiritual needs.

After the referral is made, a Novel Hospice Care case manager will meet with the patient and their loved ones to discuss available programs and services, and answer questions that they may have regarding hospice care and their particular situation.
Referrals to hospice may be made by anyone, including but not limited to:

- Hospital Discharge Planners
- Social Workers
- Patients
- Family Members
- Nursing Homes or other Long Term Care Facilities
- Friends
