
Continuous care is short-term intensive hospice care with a higher level of nursing care designed to provide relief from acute pain and symptoms. Continuous care usually happens during a medical emergency or crisis situation. This could be “round the clock” hospice care by the designated crisis team. The continuous care team can remain in place (at the home) until the medical crisis/emergency has been resolved. Continuous care can be opted in lieu of moving to an inpatient facility.
